Study Objectives / Outcomes
To analyze data from the Improving Outcomes for Multiple Morbidities Using Collaborative Group Clinics conducted at the Houston Veterans Administration Hospital. This data will be analyzed at UTMDACC, no UTMDACC patients will be enrolled. Baylor College of Medicine has a subcontract with UTMDACC to recruit and enroll patients for this pilot study. BCM is the IRB of record for the study. All study participants will be approached and enrolled through the Houston VA Hospital exclusively. Data collected from group clinic patients will only be taken from the Michael E Debakey Veterans Administration Hospital and no other sites, including no UTMDACC sites.
